Please note that travel with MHT requires completion of the following forms.

To participate in a tour, please register using our website and make a deposit, and then complete these forms which can be emailed back to us (they are fillable PDF forms.)  Please email to forms@miltours.com

If you registered online, you do not need to complete a tour registration form.  If you want to register via a paper form, click here.
